SOC OASIS
The brief mental status assessment in the OASIS must be completed without exceptions.
Rationale: Required for comprehensive patient evaluation and regulatory compliance.
We have been noticing that this part of the assessment in the OASIS is not being completed.
Complete Required Forms for SOC OASIS:
Consent must be signed by patient/MPOA.
Individualized Emergency Preparedness Plan must be completed and signed by patient/MPOA and you.
TUG score must be completed.
Braden Scale Risk Assessment must be completed.
Handle SOC Cases with No Answer Promptly: For SOC cases where there's no answer, call the office right away. Do not put it off.
Rationale: Delays can lead to dangers like patient falls in the home.
Tip: Document the call and any follow-up actions immediately.
DC OASIS
Any discipline that will be doing the DC OASIS must be the one that will complete the NOMNC.
The NOMNC must be explained and signed by patient/MPOA on your visit prior to your DC visit. Essentially, the day that you are informing the patient/MPOA that you will be discharging them on your next visit, will be the day you let them sign the NOMNC.
